The General Data Protection Regulations require that a Subject Access Request can be requested by email. As your medical record is also confidential we will be unable to complete any SAR without proof of identity.

The link below, SAR request form, enables you to print off the required SAR form to bring into the Practice with photographic proof of identity.

You may find it more convenient to register for Patient Access which will enable you to view the data we hold on you at a time convenient to you.  This service also enables you to order repeat medication on-line, have your prescription sent electronically to the pharmacy of your choice and book GP appointments.  To register for this service click this link to Patient Access.
